Mountain CAP of WV, Inc., a CDC's In-Home Care program is an array of services provided on a part-time or intermittent basis to individuals who are medically and financially approved. Clients are accepted regardless of race, creed, age, sex, or physical handicap.

In-Home Care is usually less expensive than hospital and nursing home care
Services are provided by trained and qualified Direct Care Workers. All workers are CPR and First Aid certified, and receive continuing education each year.
Our Direct Care Givers, under the supervision of our registered nurse, can assist with complete personal care needs. Some functions may include: shampoo/ hair care, bathing/ dental care, routine skin care, preparing meals, dishes/ laundry, light housekeeping, transportation for medical appointments and out-patient medical treatment, grocery shopping, and community activities.
To qualify for Title XIX Medicaid Aged and Disabled Waiver Program:

Be 18 years of age or older

Be approved by WV Medical Institute as medically eligible for nursing facility level of care

Meet Medicaid Waiver financial eligibility criteria by WV Department of Health and Human Resources

Be willing to participate in the Medicaid Aged and Disabled Waiver Program as an alternate to nursing care facility
VA Eligibility

Must be a Veteran

Must go to Clarksburg VA or an associate

Must score at least 40 points on the eligibility criteria form. Applicant will be expected to answer questions such as: Does client live alone? Does client need assistance with: continence toileting, bathing, cognitive impairment, mobility, meal preparation, feeding, medication management, etc.
Personal Care Services

A child or individual who has a physical or mental impairment that can be expected to last not less than 12 months

Be willing to participate in the personal care program and follow all guidelines.
